7. Click on “Next of Kin/Sponsor” at the top of the form, fill in the form, and click on “Save Relations”.
8. Click on “Upload Image” at the top of the form, select and upload your scanned passport photograph, then click on “Save Image”.
9. Click on “Print My Form” at the top of the form and print a copy of the form for your record.

Note that the screening exercise is scheduled as indicated on the screening exercise slip you printed out during your online registration. The date, time, and venue of the screening test will be indicated on the test slip, and you are required to take the test on the scheduled date and time.
